Endoscopic fenestration of intraventricular arachnoid cyst

Diagnosis: Intraventricular arachnoid cyst
  •  14-month-old girl
  • Acutely unwell after a fall
  • Bulging fontanelle, sunseting eyes, bradycardia
  • Macrocephaly which had not been investigated
  • CT scan: hydrocephalus, cystic lesion in right lateral ventricle
  • Endoscopic fenestration
  • Shunt placed afterwards
EQUIPMENT USED
  • Storz Lotta endoscope
  • Bipolar coagulation electrode
  • Sharp click-line scissors
SURGICAL TIPS
  • Cyst wall shrunk with bipolar diathermy
  • As big a fenestration as possible
