Diagnosis: Hypothalamic hamartoma
  • 16-year-old girl
  • Refractory gelastic seizures
  • MR scan: hypothalamic hamartoma
  • Combined microscopic/endoscopic excision
  • Seizure free at 3 years
EQUIPMENT USED
  • Aesculap Minop endoscope
SURGICAL TIPS
  • Craniotomy, interhemispheric approach, callosotomy, access in to the right lateral ventricle
  • Once inside the lateral ventricle, the endoscope is used and advanced in to the third ventricle though the foramen of Monro
  • Removal of the hamartoma piecemeal with the endoscopic biopsy forceps (the operation was performed before the advent of endoscopic CUSA)
